If you are calling about forms being late, please email us with the information and we\u2019ll get back to you. If you have additional questions, the best way to reach us over the summer is by email.\u00a0Student insurance and waiver information may be found on the student accounts website at <a href=\"http:\/\/studentaccounts.vassar.edu\">studentaccounts.vassar.edu<\/a>.<\/p>\n\n\n\n<p>If you need to receive a copy of your immunization records or medical records \u2013 please send us an e- mail including your name, date of birth, year of graduation, where you would like them sent, and exactly what you need \u2013 complete records or something specific.\u00a0In addition, we will need you to sign an authorization for the release of information form.\u00a0This form can be found under downloads on the first page of our website \u2013 <a href=\"\/health-services\/\">offices.vassar.edu\/health-services<\/a>.
