If you have additional questions, the best way to reach us over the summer is by email.\u00a0<strong>Our preferred way of receiving the health form this summer is either by email (<a href=\"mailto:health@vassar.edu\">health@vassar.edu<\/a>) or Fax <a href=\"tel:+18454377135\">(845) 437-7135<\/a>).\u00a0Please keep the hard copy for your records.<\/strong>\u00a0<\/p>\n\n\n\n<p>Student insurance and waiver information may be found on the student accounts website at <a href=\"http:\/\/studentaccounts.vassar.edu\">studentaccounts.vassar.edu<\/a>.<\/p>\n\n\n\n<p>If you need to receive a copy of your immunization records - please send us an email or faxed request with your name, date of birth, year of graduation and\u00a0that you are requesting immunization records. We can then email them\u00a0directly to you. If you have the insurance offered through the college there is no charge. They have quite a bit of information on their site. They also have a live chat area that you can utilize.<\/p>\n\n\n\n<p>How to Print \/Download an Insurance ID Card<br>1.
