To download a copy of the health form, you can access this from our main webpage. If you need to advise us about a form being late - please email us with the information and we'll get back to you. If you have additional questions, the best way to contact us is by email. Health insurance\/waiver information may be found on the&nbsp;<a href=\"http:\/\/studentfinancialservices.vassar.edu\/billing\/\">Student Financial Services website\/<\/a><\/p>\n\n\n\n<p>Our webpage: healthservice.vassar.edu also has information which may help you with any questions you may have regarding billing, services offered, etc.<\/p>\n\n\n\n<p>If you need to receive a copy of your immunization records - please send us an email or faxed request with your name, year of graduation and&nbsp;that you are requesting immunization records. We can then email them&nbsp;directly to you.
