Requesting Letters of Recommendation

In order to collect letters of evaluation for an application to a professional school you must have a file open with the office. If you need to open a file please log in to Handshake to schedule an appointment with the Office of Pre-Health Advising.

Confidentiality Waiver (for those opening a file)

Medical/Dental School Letter Registration Form—this form is to be used to register writers for the first time.

REMINDER Medical/Dental School Letter of Recommendation Form—this form is to be used to remind people for whom you have already registered for a letter of recommendation.

UPDATE Medical/Dental School Letter of Recommendation Form—this form is to be used when you have asked a recommender to revisit their original letter of recommendation. The writer should ALREADY have a letter on file.
