Instructions for Newly Enrolled Students

In order to fulfill the medical requirements for all newly enrolled students, please go to the patient Health Portal. You will need to sign in using dual authentication through Duo Mobile and then enter your date of birth.

1. Update any information that is needed under the Profile tab.
2. Proceed to the Medical Requirements/Forms section and follow the instructions on the page.
3. Be sure to update your immunization record only once and manually enter all vaccination dates.
4. Complete all forms and provide documentation of a physical exam performed within the past 2 years.

Minors (students under 18 years of age): Please find the following forms under “Annual Policy Forms”, download them and have a parent/guardian complete them on your behalf. Then upload the forms for our review. You should not complete the associated forms that are intended for students 18 years of age or older.

2. Financial Responsibility Policy for Minors
3. Privacy Notice for Minors
4. Telehealth Consent for Minors

Note: If you previously submitted documentation through our formal portal, you may need to complete some additional forms in this new system. Please check the new portal at healthportal.vassar.edu to determine any outstanding requirements.

All documents will be reviewed and validated by our Newly Enrolled Student Clearance Team. You can track your compliance status on the home screen. Once all requirements are satisfied, you will see this reflected on the homepage.
